Job Title: Job Coach
Agency: Mount Sinai Medical Center

1. Responsible for a caseload of approximately 18 consumers.
2. Responsible for all monthly VESID reports and monthly reports for consumers that reach Extended services.
3. Responsible to develop with consumers and Extended plan of treatment which will be documented in the vocational file.
4. Liaison between the consumer and all the appropriate support network.
5. Training of consumers on the job or offsite.
6. Act as an integral member of an interdisciplinary team of vocational and treatment staff.

Knowledge, Skills and Abilities Required:

1. Understanding of recovery model of rehabilitation.
2. Knowledge of psychiatric illnesses.
3. Good organizational skills and the ability to negotiate with employers and consumers.
4. Ability to work with others in a team.

Required Education and Training:

1. H.S. Diploma
2. Experience in mental health field
3. Spanish- speaking a plus
