Job Title: Full-time Peer Advocate (2 positions)
Agency: F·E·G·S - L.I.F.E. Program

2 FULL TIME PEER ADVOCATE FOR L.I.F.E.'S ENTITLEMENTS ADVOCACY PROGRAM (LEAP)

F·E·G·S-L.I.F.E. Program is accepting applications for 2 Full Time Peer Advocates. The Advocate's focus is to work with people in crisis, newly entering or re-entering the Suffolk County Mental Health service system. Advocates provide comprehensive and concrete delivery of case management services to participants; provide case specific linkage, monitor and client advocacy; as appropriate to consumer needs. The job duties must be performed in a manner in keeping with the Mission Statement of F·E·G·S-L.I.F.E. Program. The L.E.A.P. Peer Advocate must also project a positive image of the employees, participants and agency of F·E·G·S - L.I.F.E. Program to the public, participants, other agencies and families. The L.E.A.P. Peer Advocates report the Asst. Supervisor and Coordinator/Supervisor.

  • Working and applicable knowledge of Suffolk County Mental Health System.
  • Possess' an advocacy and empowerment philosophy.
  • Meet and establish relationship with consumers to assist consumers with assessing their needs in all areas.
  • To ensure appropriate service, treatment and support are received.
  • Developing and implementing appropriate treatment plans with consumers for caseload assigned.
  • Maintains a working knowledge of intra-agency and community resources.
  • Refers and/or accompanies consumers to appropriate programs, agencies and community services and maintain linkages.
  • Monitors quality of services received and advocates for consumers.
  • Maintains case records detailing service needs and activities/arrangements for their fulfillment.
  • Provides crisis assistance (i.e., referral for emergency shelter, domestic violence services and emergency food…) to assigned caseload and to other clients.
  • Conducts formal case conferences and attends meetings with relevant staff, as appropriate.
  • Provides consultation and referral services to family members of eligible consumers.
  • Develops and maintains a comprehensive resource file of both specialty and generic services for consumers.
  • May be assigned other tasks and duties reasonably related to their job responsibilities.

Qualifications:

The qualifications for this job are the ability to perform all functions with or without reasonable accommodations, experience in advocacy and working with persons with psychiatric disabilities preferred. Driver's license required. Possess good writing, reading and verbal communication skills.

Training:

Staff development supervision and training opportunities are provided regularly through in-service training and utilization or our IPRT program.
