$50 million available in Systems Change Grants developed to improve supports for Americans with disabilities or long-term illnesses from HCFA the US Health Care Financing Administration. HCFA invites states and others to design and implement effective and enduring improvements in community long term support systems.
Four distinct grant solicitations:
- Community-Integrated Personal Assistance Services and Supports Grant
- Real Choice Systems Change Grants
- Nursing Facility Transitions Grants
- National Technical Assistance Exchange for Community Living Grants
Examples of use of a C-IPASS grant:
Part A - Examples of Building Blocks - activities that could be implemented that promote consumer choice and consumer control in personal assistance services.
- Job Bank - facilitate match-ups between workers seeking jobs and consumers seeking to hire - conduce pre-employment background checks
- Human Resources Support - assist consumers with personal assistance services including tax withholding, worker's compensation, criminal record checks, etc.
- Back-up Support - temporary support from back-up workers should a scheduled worker become unavailable
- Paraprofessional Staff Recruitment, Retention and Training Efforts - as personal assistants
- Nursing Delegation - delegation of appropriate tasks to personal assistants, family members and consumers - creates non-professional jobs to relieve professional nurses
